Every paste starts at 100 and loses points once per type of problem, using the table below, which was written before any product was scored. Fluoride is recorded but never scored, because you use neem and fluoride pastes as complements. Neem sits outside the grade: a paste with no neem on its label is sorted to the bottom, and one that names neem on the pack but lists it last is marked "trace neem". Labels that couldn't be verified show no grade.

About the data
Labels were checked in two research sweeps on Sep 28, 2026, preferring brand pages, then retailer pages, then ingredient databases. Stock was then checked directly on brand sites, Walmart, Target and Kroger. A green store dot means an active listing was found; "stock not checked" means the store lists it but in-store stock couldn't be confirmed (Sprouts, Whole Foods and Amazon block automated checks). TheraNeem toothpaste, Desert Essence's neem pastes, M3 and Neem Tree Farms were out of stock or sold out on Sep 28. Neem position is a rough dose signal only: no brand states a neem percentage, and ingredients under 1% can be listed in any order. Evidence: fluoride has strong evidence for preventing cavities. Neem has lab evidence against cavity bacteria and a few small, short trials showing less plaque and gum inflammation; it is not proven to prevent cavities. Not found at Costco, CVS, Walgreens, Trader Joe's or Aldi. Not listed: Patanjali gels (India only), Dabur Babool, Colgate Active Salt Neem and TheraNeem Kids, whose US labels couldn't be found. Store prices vary by location.
